I've been having a similar problem.  In my case all the standard settings stuff showed, but my custom settings control was not showing, just ignored.

This is a module that has undergone a lot of versions.  I uninstalled it - I chose to delete files, but I doubt that was significant.  I re-installed and it just worked fine as it should do.
