Mike,

I am experiencing a similar problem.  I am currently running 5.1.2 with the shipped provider (AD).  It is a new install and not an upgrade.  My environment forces me to use Secure method and all checks pass except Root Domain.  Integration seems to work as it created my user accounts and syncs the roles properly and it displays the user name on the first visit to the page.  Any subsequent postbacks gives them Register|Login.  I am using forms authentication and have modified a few settings in order to get 2 DNN applications to work together.  I also have a 5.0 site running with a very similar configuration, and it gets the Root Domain failure as well but everything functions fine.

The forms ID for the 512 site is .DOTNETNUKE2 while the 5.0 site is default.

The 5.1.2 site is not using impersonation, the 5.0 site is ( i have toggled this with no change ).

Any ideas would be appreciated.

Just wanted to add, that my account (which has the host bit set) works fine from my PC all the time.

I have the same issue, except it was recently upgraded for its 5th year maintenance.
