I was searching for a DIFFERENT problem with a 5.1.1 installation and came across your post. Not sure if this is related or will help, but in a KB article for our web host (Crystalech), there was a note about DNN upgrades and to be sure to copy the MachineValidationKey and the MachineDecryptionKey from your original web.config to the new one. It states "This has to be done or you will not be able to login."

Thought I'd toss it out in case it helps you or someone else ...

I had performed all of that and more...

After doing a new install of 5.1.1 and examining a ton of tables in detail and comparing them to my upgraded database tables I found the issue.

For whatever reason there was a reference in the Tabs table that had a tabname of "Login".  It has the "IsDeleted" flag set to 1, and the TabPath was //Login and the IsSecure flag set to True, and the IsVisible flag set to 1.  All other fields were either blank, 0, or NULL.

How do I enter this as a bug report?  It seems that there is an issue with the way DNN 5.1.1 handles login URLs where there is a Tab ID in that uses the same name as the Login page.  This didn't appear to be a problem in the older versions of DNN.

You are not using a third party login module?
You can log the issue in detail at support.dotnetnuke.com

Thanks....

Okay, I will log the issue.  FYI machinekey and all that was not a problem...  The problem was the tabs table and the data that was in it.  And no I am not using a third party login product.
