We had found a underscore issue on uploading files, and fixed it for the release version. 

Symtome:  upload of a file beginning with a underscore worked without issue, but the images are not viewed on the gallery.

Solution for the beta:  delete the underscores at the beginning of the files.

Let me just try to answer your questions.

1. Internet Explorer 7.0.5730.13
2. Cookies are nor disabled or restricted.
3. After clicking Add File, the file appears in the pending uploads list. I then click on Upload Pending Files. Feedback is "Files were successfully uploaded and added to the album", but when I look in the folder on the server, it's not there. File was not uploaded.
4. Is Private is not checked. Auto approval is checked.
5. I have not changed this, so this is default setting.
6. I have not changed this, so this is default setting. However for the Gallery, I use 50000

There is also a restriction for file upload on the web.config file, which is not able to handle it with the Gallery at time.  Check the http handler section in the web.config file, it is setting up by standard to 8MB, if you want to setup with a bigger size you ahve also add a exceptionTimeout="1500" to it. 

Are you logged in as Admin, host or as a user?

I'm logged in as an user with Admin rights.

Actually I just made a new discovery. I can add multiple pictures to the first album I created. The pictures are around 700-800 kb each and there is no issue in timeout/uploading. But when I try adding picture (s) to album nr 2, then it would not let me.

I just realized that I have actually 1 album and 2 subalbums and my problems are with adding jpeg images to subalbum #2. I did not know that when I use the gallery module then allready there it's an album.
