The formandList website is not sending out email notifications of when you sign up. I have the mosule but want to know how to upload the simple form template? What is the instructions for this thanks

i have uploaded it to the templates folder. And I know its the right folder because I can save exported content and it shows up there. How do I get it to pick up the template?
