The logic in RoleController.AddUserRole is such that if the Effective date parameter is Null, which it is in many cases, a Null is stored in that field, regardless if the Role is new or being updated.  This is wrong - if an existing Role is updated, and it has the Effective Date set, it will be replaced by Null, wiping it out.  It should be preserved.  If the Role is new, today's date should be used.
