I am sure there is any wrong on running the upgrade.  I running the forum on production environment and testing the module on serveral DNN Versions as a new install and as upgrade without any issues.

Make sure that the user of your application pool have full access before you install the Forum.  A upgrade of a Hosting application by the hoster could change this settings.

Check the event viewer in administration section for more issue information and post it completely.  Does you have any issues on upgrade? Have you other modules running on same page ?

 Basicly on a hosting there is a webapplication (i.e Plesk, DotNetPanel, WebsitePanel) availible where you can setup the permissions.  Notify that Plesk firsat supporting DotNetNuke installs into the root folder since Plesk version 9.5.1.

I have a production site (v4.9.4) with forum v 4.5.3 on a shared hosting provider - same issue (My hosting provider does not use any of the above control panel apps and will not make the changes suggested to folder permissions)
I downloaded a backup of the sites database, set up a new version of the site on my server, checked the application pool identity and IIS User had the required permissions, upgraded the site to v 5.5.1 - same issue remains.
I then set up a new install with the app pool id permissions as stated - same issue remains.

I've now turned the members list feature off in all forums until the team can come up with a suitable explanation.

I am not sure if we should discuss this issue about the Forums UserList feature ( outside the Admin panel) longer about it would not longer part of upcomming versions.  Upcomming Forum Versions no longer included PM and would also no Userlist included for the users.
