I have two instances of form and List module 5.0.3 version displayed on the same page. The first instance is configured as a form, the second instance is configured as a list, both instances use the same columns. How do you link these instances together so the list instance shows all the records submitted via form instance? Currently, the list instance is empty, showing no records. Thank you.

FnL5.1. (currently in release tracker) offers a form and List mode.
Meanwhile you can only use the reports module with the FnL datasource to show data from one instance multiple times on one page.

Just trying to get more clarity on your reply. Are you suggesting using version FnL 5.1 over 5.0.3 (which offers form and List mode as well) ? Also, are you saying to use a report module instead of list instance of FnL to display all records submitted in the past? Thanks again.

form and List 5.0.3 offer to use  form or list mode, FnL 5.1 will include both.

FnL5.1.1 has not yet passed the release tracker, a release candidate is available at codeplex.. The reports module might be a viable solution for your needs, though I don't know your exact requirements.
