The craziest thing just started happening over the past couple of days with our DNN 4.8.2 intranet. With Authentication set to Windows in the web.config, new users are not being created but all is well with thier role based access to links and modules. With Authentication set to forms, users system wide are not seeing (all) thier role based links and modules but new users are created and role assignments are working normally. I am sort of between a rock and a hard place.

Not sure why this started happening, but upgrading the AD provider from 1.00.04 to 1.00.06 doesn't seem to matter. I plan to try an upgrade to 4.9.5 since we don't want to purchase DNN.

Is there anything I should try before resorting to a full core update?

Very odd. Have you always run DNN under Windows authentication in the web.config? I'm afraid I don't have a lot of experience running under Windows authentication as I've always run under forms.

I don't think an upgrade will fix the problem as Windows/forms authentication is a .NET framework setting and not a DNN setting but that said, it can't hurt to update just for security/bug fix reasons. I'm confused though why you say you plan to upgrade to 4.9.5 because you don't want to purchase DNN. DNN 5 has the totally free Community Edition still so there's no cost to you there.
