Is it recommended to upgrade production site to new DNN version 5.2.1?

The standard recommendation is to upgrade to the most current production, stable release of DotNetNuke.

5.2.1 is the most current release and many people are upgrading to it without issue.

The actual decision to upgrade is based on your needs, site configuration, experience with the upgrade process, etc.

There were few know issues with new version of DNN. For ex:

  1. On upgrading, the caching providers give an install error as vr1001vr1001. These install errors are caused by some issue with updating web.config.
  2. Bugs related to multilingual text in HTML module and other places.
  3. Sites that have modules or skin objects that use the Telerik controls have some issues.
  4. Issue on encoding of cached items.

Are these issues resolved in new release?

AFAIK these issues are fixed, there is a known upgrade issue, if you are using a database owner other than "dbo" or a non-empty object Qualifier in your database.

If we upgrade to new version of DNN, what happens to already created pages, modules and contents of the existing portals? Do they remain unaffected?
