I apologize if there is already a thread for this, but after an hour of searching I could not find one that matches or has a fix for my issue.
 
I have DNN 5.2.2 and am using feedback module 4.4.3.  IIS 7 is set up to use localhost, for SMTP, and my SMTP tests work. In addition confirmation emails for such items as adding a user account work as well.  I have added the feed back module to a page and have tried various configuration settings.  However when I click on the send link, nothing happens. If I delete a required field and click send, I do receive the validation errors.  However if I enable Captcha, and enter a wrong security code, after clicking send, nothing happens.  I do not even get a warning message that I have entered the wrong security code and to try again.  Also messages are not even showing up in the moderation module.
 
I am using admin@mydomain for both host and admin accounts. Anyone have any ideas?

Problem solved. 

I am using the module as a “contact us” form on our contact us page.  The page consists of 3 HTML Modules in various panes, the Feedback module, and a Map module.  After trying to get this to work for a day and half, I decided to try creating this on a page that was not live.  When I created a new test page and place the module, everything works as expected.  I tried deleting the Map module on the live page, but that did not help.  So I recreated the contact us page, and made sure I placed the feedback form on as the first module.  Once it was configured and working, I placed the HTML modules and tested, then I place the Map module and tested.  It all works now.
 
There may have been something with that particular page that prevented the module from working.  Maybe it had to do with placing the Feedback module on after the others, maybe not.
 
If you have a similar problem, try recreating the page starting with the Feedback module.
