Is there a css or something similar we can customize to get the form to look exactly like we need?

I don't know whether it can be customized to look exactly like you want.

Thanks, that should be plenty. One other question regarding the form, is there any way to get rid of the "*" on required fields?

Of course. The required indicator (*) is a string and therefore also localizable using the language tools inside DotNetNuke. The string you are looking for is inside the resource \DesktopModules\UserDefinedTable\App_LocalResources\sharedresources.resx
