Hi Christian, Return the origial installation before the upgrade (database and directory dotnetnuke) and run the upgrade to 5.4.0, inspect .zip files in directories "install/modules" and "install/languages", in this case, remove the .zip files before you begin the upgrade.

You have deleted all .zip files in directory "install/modules" before starting the upgrade?

Yes, all .zip files were deleted. Can you please describe the process more detailed?

Christian,

The error listed is: System. SqlClient. SqlException: Violation of primary key constraint ' PK_TabModuleSettings '. Cannot insert duplicate key in object ' dbo. TabModuleSettings ', i.e. a violation in ' PK_TabModuleSettings '.

Most probably you are inserting a module that is already installed.
I had a similar problem when upgrading (5.4.0 from 5.3.1) because there were some modules (zip files) in my "install/module"  directory.
Everything indicates that you are having the same problem. After deletion of the files my upgrade worked without  problems.
Be sure to be using the database version 5.3.1 and that the installation directory is version 5.3.1. You can do this by accessing your DotNetNuke Portal as SuperUser and selecting Host/Dashboard and clicking on the "Host" option.

See below:

This is the directory:
