I had the exact same problem.  See:

This is what I did to fix my issue on my localhost DNN site that I use for testing upgrades.

I downloaded the 5.4.1 Upgrade zip file.  Once on my PC, I right-clicked the zip file and selected "Unblock".  I've never had to do this before but this was the key to getting it to work.

I then deleted the rows in ther version table for versions between the version I was upgrading from (in my case 5.2.2) - and 5.4.1.

I then unzipped the files to a temp directory - then copied them into the website folder.

I then browsed to the site and the upgrade worked.  (I did get an error on one of the SQL scripts - but this is because the script had already run).

After this, everyhing was OK.  If this is a Production website, I'd strongly suggest making a copy of it and testing this to make sure it works for you.  I always test upgrades before applying them to a Production environment.

I have see this in an install of mine and I will try to track it down. For the time being it is stopping me from doing additional module installs, not sure about skins yet.

I don't think it is the same issue. This one allowed the install to proceed and recorded the probable file access issue to the event log. Most definitely some files are not deleted. The "RemoveOrphanedFile" gives a clue to this.

I've experienced a side effect with module installs failing after the temp file cannot be removed. I raised the issue internally and Alex already generated a bug report at Gemini.

Thanks for the feedback.

I'm going to abandon the upgrade and hold out for a fix.

Caroig - if you can please try the upgrade again (maybe stage this) - it could be a race condition. We would be interested in any feedback....
