1. Upload 1-2 files to the "Root/TestFolder/img/"->   Image Editor (refreshing)

2. Image Manager - > on event (scrolling file list )

The target 'RadFileExplorer1' for the callback could not be found or did not implement ICallbackEventHandler.

3. File has been uploaded, but not present in the folder (Refreshing does not solve this problem).

4. Admin -> File Manager : Select folder:  Root/TestFolder/img/

In additional:

I tried to resolve this, and I found somthing very intresting .

SELECT top 3[FileId]  ,[PortalId]  ,[FileName]   ,[Extension]     ,[ContentType]      ,[Folder]   FROM [Files]  Order By [FileId] Desc

Result:

797    0    img_thumb.jpg                                                                          jpg    image/jpeg    TestFolder/img/
796    0    CC_O_thumb.gif                                                                       gif     image/gif        TestFolder/img/
795    0    C:\Documents and Settings\Evgeny\Desktop\b002.jpg    jpg    image/jpeg    TestFolder/img/

Looks like TelerikEditorProvider set wrong filename (Path to the uploading file,  instead of file name).

All I could reproduce was this issue:

If you can find another issue, and can supply us with full step by step instructions (right from the beginning from a DNN clean install, that includes documenting the upgrade step if required, so there are no problems reproducing the issue(this would require some testing yourself) ) then please log at:
