Fortunately, this exercise was done on our test server instead of production.

Any ideas on how to debug and get past these issues?  There are no errors in the system event logs or the Providers\DataProviders\SqlDataProvider

Ok, after 12 hours of banging my head against the wall, I got it. (yay)

All clues eventually pointed to a problem updating the web.config so I pieced together a new 5.4.1 config from the Release.config and all of my previous changes.  After getting DNN 4.5.1 to half-ass work, I tried loading the problem modules & problems and received this message on some of them:

Info Creating backup of previous version - web.config
Failure Exception - Object reference not set to an instance of an object.
Failure Installation Failed - Config
Info Rolling back component install - Provide
...

I then found the following post which gave me the answer:

Basically I just need to comment out the <Location> element from the web.config file.  I had added this nested element a while ago to get a virtual application to work under the DNN web site.
