I am using Ifinity URL Master.  I've upgraded to 2.0 and it did fix the "add module" issue.

But now my images that are in the media module are not displaying.  Am I missing something with the new version of Media?

To bring closure to this issue:

Yael did contact me off-forum and provided remote access to his site's database. Although he indicated that his site did not use iFinity URL Master, I did find that it had been installed at one time and had placed an insert trigger on the Modules table. As part of the trigger code which would run everytime a row was added to the table there was a reference to the ModuleTitle column which was removed as part of the DNN 5.5.0 upgrade. Unfortunately, when URL Master was uninstalled sometime in the past, the trigger did not get disabled or removed thus resulting in this issue. Since the trigger only referenced the ModuleTitle column and not the Header or Footer column which were also removed in the 5.5.0 upgrade, those two columns did not show up in the error. I have disabled the trigger and am waiting confirmation that the issue has been solved.

It is my understanding that iFinity has released a new version of URL Master that is compatible with DNN 5.5.0.
