My main question is: does anything in the Portals folder get updated/upgraded when applying a DotNetNuke upgrade from 4.9 to the latest DNN version. The reason I ask is, if I have to recover to a previous install of DNN using FTP, can I upload everything from the backup apart from the Portals folder (a big folder) and then just restore a previous backup database?

I don't know of anything in that folder that gets changed during an upgrade, but it is definitely possible. There are some new features with the upgrades that put things into the Portals directory, but I think those occur after the site is up and running. One thing you might do is before upgrading try to copy the Portals directory, then if you need to restore, just rename that copied directory
