Are you sure, you are using FCKEditor, or TelerikEditor (the one used here, new default in DNN 5.5.)? There is an improved Telerik Editor provider available for free download at radeditor.codeplex.com.

go to File manager in Admin menu and in top right corner perform a full recursive file sync.
