Issue 1:
To must be a valid e-mail source, it can't be a simple string. If your users are registered dnn users, I suggest to use simply [created by].  otherwise the input field e-mail must be of datatype email. You might default it to @yourdomain.com, tthat way your user just need to prefix his user name.
issue 2:
- Set Appearance to "Form" or  "Form above List"
- In form settings, set Action upon submit to "show text" and Displayed text after form post to Thank you for using the Online Maintenance request Form. Our team will contact you shortly
Issue 3:
Set List view to "List view is restricted by Permission" and apply the corresponding permission
Issue 4:
All Strings can be changed within DNN localization editor. Btw, In form mode, the text for the "add button" already changes from "Add new record" to "Submit"

Appreciate your help . Please find the results

Issue No:1
The e-mail is a valid email source. However when i add the following to the To Field [field:e-mail] still does not like that. It gives the same error message. Is the syntax correct?
                            Tracking/Notification issue: Field field:E-mail is either not a valid source for email adresses or it was not found inside fields definitions.
             Defaulting is working out fine, so the user has to prefix only his username.

Help text for to:
"Enter one or more email addresses, separated by semicolon ";" or enclosed in [brackets]. You may enter the column Title name for column types Email, Created By, Changed By, User Link, e.g. admin@domain.com;[Created By];[Secondary Email]"


it is simply [e-mail], not [field:e-mail].

Hi Stefan
Appreciate your help so much.That worked out fine.

The only problem i had was that it was working with internal users(within our domain). When i used the form for external users(gmail,hotmail,etc)...it was not working. I searched the forums and found out that there is an DNN event viewer which actually helped me to find out the answer. The error was that
"NOT RELAYING" which i searched through the forum again and found out that "SMTP Server has to relay".
Added this setting in the SMTP Server and worked fine.

..Please mark this your earlier mail as the answer as i did not see th option :)
