Since when creating a module it can be displayed on every page by clicking the display on all pages box, how do you figure out which page was first on to delete -- so that all instances on every page will be deleted?

Under Host | SQL run the following query and it will tell you what modules are displayed on all pages and what pages they are on.

select * from modules left join tabmodules on Modules.ModuleID=tabmodules.ModuleID where Modules.alltabs=1

Thanks. I can see that information, too from the Add to pages on any instance of the module. However, I was hoping to be able to figure out where the initial instance of it was -- I thought that by deleting that instance all other instances would be deleted. Bottom line, the module is a distraction for visitors and I'd like to get rid of it -- if I delete the module under extensions management will that work?

if you delete a module in Host Extensions, all instances and its data will be removed.

If you run this query you can identify which module is being repeatd by the alltabs setting being checked.  select * from modules modules  where Modules.alltabs=1

Then you could do an update on the module you want to stop repeating by doing this.

Update Modules
Set AllTabs = 0
Where ModuleID=something
