We are using a form module so that our clients can register for a class online through a form.  We have set up the form module under 'Module Configuration' to send a confirmation email to the person who has registered (and their email address is a required field), and also a notification email to a staff member here, however no one is receiveing either a confirmation or notification email.   I noticed in the "Module Configuation" page, there is a link at the very bottom that says "release my license."  I don't have a license that I know of and don't know what this means - could our problem have something to do with this?

There is also a place for this data:

Send The Actual Data Submitted?
Include Text Area fields?
Include HTML Fields?
Include the user info from the DNN user profile (First Name, Last Name, Email address)?
Email Format

Perhaps we are doing something wrong here that is the cause of our problem?

Do you happen to know what "Form" Module you're using? That doesn't look to be one of the ones that comes with DotNetNuke out of the box, which likely means you would need to contact the vendor/developer of the module to get support for it.

You can find out what it is called by going to the Module Settings for the form.

Also, make sure you have SMTP configured and working on the Host/Host Settings page in order for emails to work on a DNN site.

Yes - the module is called "NS Form Builder"

Mostly it's because of the miss configuration of the SMTP under Host settings.
