Now i want to replace the token with a control type like textbox and fill the value of the control that is stored in the database

<td>Name</td><td>[NAME_CTRL]</td>
</tr>
<tr>
<td>Email</td><td>[EMAIL_CTRL]</td>
</tr>
<tr>
<td>Country</td><td>[Country_DDLCTRL]</td>

If you are talking about showing generic information from the dnn hosting database - such as current user name, email etc.
The this is doable in the standard HTML module using token replacement - if you turn on token replacement in any HTML module settings - tokens like the following will automatically be replaced.

[User:Email]
[User:Username]
[User:DisplayName]

If however, you are talking about doing token replacement then you need to build a custom module or possibly use the new DNN Razor Host interface module - but you need to understand a bit about asp.net markup to get that working.
And you need to be running on a asp.net 4.0 server for that methodology.

in you custom module, you may use TokenReplace as well, even with custom tokens, please check out source code of Announcements module, how you can achieve this.
