hello,

I've created a form (no list), and that was really easy and cut and dry.  However, I cannot figure out how to apply a custom style sheet or alter the laytout on the form.  The documentation doesn't speak to this and there is no good reource online that I cna find to apply a different template to just the form on this module.  For example, I woud like to make my form look like this one shown here on the formandlist.com site: http://www.formandlist.com/Gallery/Fe...

Can anyone help me on how to do this?  I can download the template from the site, but how and where do I apply it to the form so that it looks like the link referenced?

many thanks, mike.

I must have been blind when looking over this the first time.

fyi - the author provides a great example here on this page on how to edit the style sheet to get the same style results.... http://www.formandlist.com/blog.aspx?id=33&title=New Feedback Form
