Hi guys!

I would have to be an application which can the following:

A user logs on
He sees the other workers, clicking on one worker name, then he gets all pre-defined questions (the questions same for all workers) and
he can evaluate the worker 1-5. In the end, he may also participate in a free text.
The administrator can create a summary that how many workers assessed the others one by one, and the worker's how much the average point in the questions.

I think itt will easy in DotNetNuke, but I don't know which module can do this for me!

Please help!

I am not sure, whether this is a common use case, I haven't seen a module for this yet. Did you search for it on www.snowcovered.com?

It looks like you need to build custom module. Or to use Forms builder.
