I've faced with the same problem recently.

After analyzing an entry for the "phantom" folder in the database table [Folders] I've noticed that there was no '/' in the end of the FolderPath string.

So the problem was solved by adding the '/' symbol to the end of the FolderPath field in the database and then performing a recursive sync in the Portal's File Manager. After resynchronization the "phantom" folder disappeared both from the database and File Manager.
