I am thinking of using the form n' list module instead - as this is quite diverse,
only problem is; how to make sure they only complete the form once?

AFAIR there is an option in Module Settings (not FnL configuration) to limit the number of records per user.

yes, I believe there is a 'maximum records per user'
in settings >> form n list settings, but we don't ask users to login to use our site so everyone is anonymous.
I was thinking more of a cookie method to determine if the form has been submitted before.
