Is it possible to have another role be the default automatic role. For instance:

When  person registers they are first put on a trial registration then later they can be added as a full registered user. Also they trial users would not have access to everything that full registered users do.

I have tried but I can't find a way to prevent the registered user role from being automatically assigned to everyone who is approved for membership.

yes, you just create the role and set it to auto-assign (you can also make a role public so users can opt in and out of it) - http://www.dotnetnuke.com/Resources/Wiki/Page/roles.aspx . As to the registered users role, this is an autoassign role and new users are automatically added to it - http://www.dotnetnuke.com/Resources/Wiki/Page/Default-roles.aspx
