I think you're talking about a Sales Form, where your customer can input their information and submit the form to purchase your hosting plan. In DotNetNuke almost everything is possible, thanks to great module developers in DNN community.

There are many modules available; first you may want to try DotNetNuke (Forms and List) Core Module if that does not fit your need have a look at the OnyakTech's H20 Module. There are number of other forms modules available  at Snowcovered.com
