Patrick, the error points to a database upgrade script, which didn't run. you may run latest uprade script(s) manually using Host > SQL (scripts are located in /desktopmodules/html/providers/dataproviders/sqldataprovider folder).

 As I mentioned above, we've already done this "Executed the 06.01.00.SqlDataProvider but still it wont work" but it still doesn't work

Patrick, maybe you need to run previous script (and latest again) etc., until the module is working as expected. otherwise you will need to export content of all HTML module instances, delete the module in Host modules, reinstall (from DNN install package, folder /install/module) and re-readd all module instances and import all module content.

I've recently upgraded from 5.6.7 to 6.1.5 and am now experiencing this too!

My HTML module is saying it's the latest version, 6.1.5 and didn't show any install errors on the upgrade.  What I did see as errors on the upgrade were:

00:00:19.234 -   Executing Script: 06.01.05.SqlDataProvider Error! (see 06.01.05.log.resources for more information)

and

00:00:53.859 -   Installing Package File DDRMenu_02.00.02_Install:  Error!

The 06.01.05.log.resources file showed:

System.Data.SqlClient.SqlException: The ALTER TABLE statement conflicted with the FOREIGN KEY constraint "FK_tacuk_Blog_Blogs_Portals". The conflict occurred in database "TAC", table "dbo.tacuk_Portals", column 'PortalID'.

and then a list of tables etc but no mention of HTML.

Did anyone find a solution for this?  or why it is even happening?

It's really surprised me to find it happening, even in the newest DNN upgrade.
