Actually, I'm having the same problem and I do have several events in the event viewer (I tried to change "Survey Settings" several times).  The survey module instance is named "President" and this is the message in the event viewer:

I'm looking at the database through the web admin tool of my website hosting provider, so it's a little foreign.  On to more drastic remedies; if I were to remove the survey module and then reinstall it would that ultimately solve the issue?  What would generate null values in datetime fields?

SurveyID int 4  10  0  False      True  1  1  False  Rename Edit  Extended properties Drop
ModuleID int 4  10  0  False      False      False  Rename Edit  Extended properties Drop
Question nvarchar 500      False      False      False  Rename Edit  Extended properties Drop
ViewOrder int 4  10  0  True      False      False  Rename Edit  Extended properties Drop
OptionType char 1      False      False      False  Rename Edit  Extended properties Drop
CreatedDate datetime 8      False      False      False  Rename Edit  Extended properties Drop
CreatedByUser int 4  10  0  False      False      False
