I have a batch of forms I have previously used successfully by saving the template then applying the template on a new site.

This last time I tried to do that, it worked once, then failed to apply the template completely every time after that.  When I select the template to apply, it successfully applies the Title, Header and Permissions tags, possibly more. But it fails to import any of the fields and continues to display the table with optional templates to apply. The part that confuses me the most, is that 1 form successfully applied the template, then never worked again.

I am not sure how to fix this issue, I tried looking in the SQL, but can't understand it to see if there is a flaw (I understand tables, not what is going on in there).

This happened initially with DNN 6.0.1, I have uninstalled and reinstalled F&L 5.1.3, and upgraded to DNN 6.0.2 and then 6.1.1 and the issue does not go away.

If someone could give some advice that would be great. If any module admin types want access to the site, I can grant that.
