What does it mean when I see this at the top of my pages (only appears in the admin or host pages)

Could Not Load Skin: /Portals/Skins/Skin/page_sub.ascx, Error: Index was out of range. Must be non-negative and less than the size of the collection. Parameter name: index 

any thoughts on how to fix this?

Hi,
May I ask is this skin your custom skin? And have you correctly installed it?
Generally, a skin would be put in the "_default" or special portal id folder under Portals.
for example, the skin path will be: "/Portals/_default/Skins/Skin/page_sub.ascx"
or "/Portals/0/Skins/Skin/page_sub.ascx" or "/Portals/1/Skins/Skin/page_sub.ascx" ...

Yes it is a custom skin... I believe that it is installed correctly. Also, it only shows on the root admin and host page... it shows up nowhere else... not sure why.
