Accepted Answer 
Finally my issue is resolved now.

I found 2 records with null "ModuleDefId" and "ControlKey". I have updated "ModuleDefId" by referring table ModuleDefinitions.

Hope following query might help someone to check null values
SELECT * FROM ModuleControls WHERE ModuleDefId IS NULL AND ControlKey IS NULL

Thank you John and William for your suggestions. Your advice helped me to get this fixed.
