What the best upgrade path from an existing code base with the 4.9.5 framework? 

I have tried 6.1.5 and 5.4.4 but both fail from the 4.9.5 framework.

please see http://www.dotnetnuke.com/Resources/W... . Normally you dont have to do some many small "jumps" but if you're having problems you may want to try that upgrade path. I suspect your upgrade issue(s) may be caused by changed prerequisites so make sure you've installed .net 3.5SP1 (or .net 4.0 and changed the application pool), and make sure the database is sql 2005 or higher (sql 2000 doesnt work after 5.2.0)

FYI, I was upgrading from 4.9.5 to 5.1.3 and had all sorts of problems. Then I found the resource that Cathal posted and it worked great.
