Is there an installation/upgrade path from v3.2.2 to 4.3?  I haven't been able to find any info on this in the documentation.  If so, what version do I use? (install or upgrade).

What will break or what manual changes will I need to make if I go this route as opposed to staying with v3.x?  I have several custom modules in place right now.

You should be able to upgrade using the Upgrade package. Extract the package over top of your installation and then rename your current "web.config" (to web.config.old or something). Then rename the "release.config" that came with the package to "web.config" and update the connectionString, objectQualifier and copy the "machineKey" setting from your old web.config (this is important because without copying this, DNN can't decrypt the existing user passwords for authentication or password recovery).

That should work as far as configuration is concerned. The modules should be compatible but I would recommend (if you have access to the SQL Server) backing up or making a copy of your live DB and files then you can test the install there first. It never hurts to be safe when upgrading major versions of any software
