I am looking for a form that would allow viewers of the website to fill out the form and to submit it.  For example, I want to insert a insurance questionnaire on my site where viewers can fill out their names, address, phone #, # of accidents/tickets, etc... and then submit this form so I can look at it, and later on respond to them.

There are number of modules available like "Dynamic Forms" from DataSprings, UniversalList v4.05.03 from cookdnn etc...

I have used Dynamic Forms on my client site.

The free form and list module can do this for you.  Pretty easy to set up.  Let me know if you need help.
