I have just installed the module, and added it to a test page. When going to 'Configuration' I am getting the error below. I have tried to identify what the problem is but without success. We are running - DotNetNuke Community Edition. Version - 06.01.05 (142). 

The install has overwritten the User Defined Tables on the web site, and I am now getting the same error on those. When I look at extensions it quotes the Form and List as being an old version 5.1.2, so its almost as though it has not installed correctly. I have repeated the install wizard, but am still getting the same error.

I can add a new record to the existing (ex User Defined Table) modules

When clicking 'Configuration I get the following error -

please clear all files from /desktupmodules/userdefinedtable folder and re-install the module.

Thanks for the quick response. I have deleted all the files from the UserDefinedTable folder (should I delete the folder as well?), and installed Form and List again, but am still getting the same error.

I should mention that there does not seem to be a Form and List folder in the Desktop Modules folder?
