First question is probley an easy one.  I've got my 4 lists to show up on my registration page.

How do I get Field labels and the help text to appear?  As you can see it doesn't tell the user what to put there.

 

I'd like my lists to have 5 levels.  I've set it up in the database as such but it doesn't seem to display like that in the List mangaer.  It just shows my first 2 lists.  I eventually want a control that will limit one based off the selection of another.  Just like the Country Control but with 4 levels.
