Sebastian, the query came back with two records (remove new user status).

The user ID associated with the records are not the same as the user who is having trouble and my test user. I did try 'switch user' to become one of the returned users and questions are working fine for that user.

ObjectKey should be a unique value allowing to identify a journal entry, but unfortunately this is not enforced in the database and somehow QA managed to create a duplicate value. Please modify the value of either these entries (It doesn't matter, the value it just needs to be unique within the table) and the issue should be gone.
