I upgraded our intranet instance from 7.0.6 to 7.1.2 and now the file manager module does not appear anywhere.

Not under Host > Extensions or Host > File Manager or Admin > File Manager. It's not in the modules drop-down to add it to a page. I can see the files under DesktopModules\Admin\FileManager - but it's like the module doesn't exist as far as DNN is concerned.

Any ideas?

the module has been replaced, but it looks like upgrade procedure failed for your site.
Check Host > Extensions, section modules for "Digital Asset Management", and make sure, all sites in your DNN install are assigned.
Now module should be listed in Add Module section (group Admin or All) - otherwise you need to grant permission in Admin > Extensions.

Looking through the instance, the Digital Asset Management module never got installed in the upgrade.

What's my next step? Manually install the module?

That was the next step! I manually installed the module from the upgrade installation files. All is working.
