typically an existing project coordinator adds the new person(s) to the list of coordinators/developers on codeplex. If you cant contact the existing project coordinator or they're not willing, you can pull the source code off codeplex and move it elsewhere e.g. github. Unfortunately codeplex (at least currently) doesn't do forking as well as github where it's trivial to create a fork and create pull requests back to the original project or choose to tweak the project and release it as your own.

That make sense for Codeplex and if you are forking the project, but still does not explain how one becomes identified in the Community Forum as the project leader and/or forum moderator? Regardless of oversight and influence there has to be someone who coordinates. Right?

 In terms of the DNN forums there are a number of moderators (such as myself), who tend to be well regarded community members, DNN MVP's or DNN Corp employees.

In terms of projects, you can always look at the relevant team page e.g. https://dnndocuments.codeplex.com/tea... . Whilst this doesn't identify Mitch as the sole project lead, the other's in the coordinator role are there primarily as legacy members, or from the days when the documents module used to be shipped with DNN itself.

For projects that used to be a part of DNN, we do try to follow them and if they are stagnating request the project owner find a new project owner, but as they're independent we have no control over that. That said documents had a release only a few months ago so it's in reasonable shape IMHO.
