please check and make sure you aren't using a webgarden (asp.net does not synchronise across threads) - http://www.dnnsoftware.com/wiki/Page/... .

I checked for the webgarden settings and sure enough it was set to 2 processes so I changed it to 1 and so far the issues have gone away. I have never changed those values before in IIS so I wonder why these symptoms did not show up sooner, or perhaps no one noticed until now. 

I believe this has resolved the problems so thank you very much! I only had time to verify on one instance but it made an immediate difference. I will assume all is well and only report back if another instance still has the issues.

excellent - webgardens don't work in asp.net (though you can buy 3rd party tools which synchronise threads) and can be hard to debug as multiple requests from the same user may go to the same thread (depending on site traffic), which means the issues show up only occasionally -as the key area is when editing (as the bug only shows with changing data), webgarden issues are typically only noticed by content editors.
