The connection string says integrated security, but then in the dataprovider section is this line:

 <add name="SqlDataProvider" type="DotNetNuke.Data.SqlDataProvider, DotNetNuke.SqlDataProvider" connectionStringName="SiteSqlServer" upgradeConnectionString="" providerPath="~\Providers\DataProviders\SqlDataProvider\" objectQualifier="xxxx" databaseOwner="yyyyyy" />

I changed the actual names. user yyyyyy has db owner permissions, I double checked that, but what other permission issues could there be?

object qualifier should be "dev245" (not "xxx"), user account is same as configured for ASP.Net in advanced settings of the Application.

Actually, object qualifier is dev245, I don't know why I masked out the user and the oq. xxx was just a placeholder. That aside, I don't know what you mean by "user account is same as configured for ASP.Net in advanced settings of the Application." Is this something I have to do in IIS manager under the application pool?

if you are using integrated security in your connection strings, you need to configure the ASP.Net user (specified in advanced settings of the IIS App Pool) as Windows account for SQL Server and grant dbowner permission for your DNN database.
