if you stored it in Header and Footer inside module Settings, it should be stored in TabModules table.
if you used dedicated fields of a module, it will be in the module's table.

In DNN 7.4 I found the information via the following SQL Statement.

SELECT TOP ( 1000 )
ModuleDefinitions.FriendlyName AS Module_Name ,
Tabs.TabName ,
TabModules.PaneName ,
TabModules.ModuleOrder ,
TabModules.Header ,
TabModules.Footer
FROM TabModules
INNER JOIN Tabs ON TabModules.TabID = Tabs.TabID
INNER JOIN Modules ON TabModules.ModuleID = Modules.ModuleID
INNER JOIN ModuleDefinitions ON Modules.ModuleDefID = ModuleDefinitions.ModuleDefID
WHERE NOT ( TabModules.Header IS NULL ) OR
NOT ( TabModules.Footer IS NULL );

I found the simplest way to find the information was to use a SQL query, as I had too many modules/pages to search through manually.

SELECT TOP ( 1000 )

        ModuleDefinitions.FriendlyName AS Module_Name ,

        Tabs.TabName ,

        TabModules.PaneName ,

        TabModules.ModuleOrder ,

        TabModules.Header ,

        TabModules.Footer

FROM    TabModules

        INNER JOIN Tabs ON TabModules.TabID = Tabs.TabID

        INNER JOIN Modules ON TabModules.ModuleID = Modules.ModuleID

        INNER JOIN ModuleDefinitions ON Modules.ModuleDefID = ModuleDefinitions.ModuleDefID

WHERE   NOT ( TabModules.Header IS NULL ) OR

        NOT ( TabModules.Footer IS NULL );
