Our Notifications are using the standard unmodified email template. The profile image isn't displaying. It shows as a broken image. The path it uses is: http://[mysite]/DnnImageHandler.ashx?mode=profilepic&userId=22&h=64&w=64

 When I put that into the browser directly, I get a: 403 - Forbidden: Access is denied.

 So it would appear the DnnImageHandler.ashx is locked down in terms of permissions and that's causing the issue.

I took the drastic and unprecedented step of reading the documentation, https://dnntracker.atlassian.net/secure/attachment/29400/DnnImageHandler%20Documentation.docx

And found that there's a setting, AllowStandAlone, that, when set to true, resolves this issue. Seems it might be wise to set that to true for all installations.

This wasn't fixed properly in the release. They didn't address the issue with the AllowStandAlone being false thus preventing access to the file with a 403 Forbidden error. In order to fix this, you need to add the following line to your web.config in the section:
    <add key="DnnImageHandler" value="AllowStandalone=true;" />
