I am having the same problem too... the AD problems just seem to never end.
Scratching around in the database, here's what's happening for ME:
I login as DOMAIN\USER, works great - my name, email, everything gets pulled over.
I try logging in as USER, it creates another account as USER, but doesn't pull everything over... except for e-mail, oddly enough. So my first and last name is NULL, which sparks the below error. Filling NULL first/last name fields, and any other required fields, should get it working.
But as for why it creates two accounts, pulls over the e-mail address but nothing else... sheesh, I have no idea at this time.

Then I go and look for the new user, which I think should come up in User Account. But there actualy is none and if I click on "all" to show them up I get the following error: "Error: User Accounts is currently unavailable."

Which lead me to the thought, that he can't only access this user record. Because if I search for any other words I don't get any errors. Even when I click on the First letter where the user should be listed I get this error and none on the others.
I tested this with a clean Source installation of DNN, version 4.3.2.
The user I use for reading the AD is a simple normal user, but I thought this should not be the problem?!
I tried it about 3 or 4 times with clean installations and nothing changed.
So, now has anybody suggestions on workarounds?
