I have been a victim to this quirk with the document module twice now so I thought I'd post this for anyone else who gets bit by it.  I have a site initally built on DNN 3.2.2 which i upgraded to 3.3.5.  I also upgraded to the latest document module 3.1.5.  It seems that either during the DNN upgrade or the document module upgrade, two entries were put into the folders table one for a flash folder and one for the media folder which were not created in the file system.  Subsequently, when I tried to open the File Manager for the portal, I would get a module load exception error.  Being new to DNN, it took me forever to figure this one out.  Once I manually created the flash and media folders, the file manager works fine.  Hope this helps someone in the future.  Can anyone explain why this happens?

This will be an issue with the DNN core (rather than the documents module).  It might be worth posting it here so it gets more exposure.
