I have set a screencast showing the crash here : http://www.admservicesonline.com/mikehingley/Errorpage/tabid/133/Default.aspx, however the flash file can also be downloaded here : http://www.admservicesonline.com/Portals/3/CAPI%20Memory%20error.swf.

At the start of the video I am just showing the toolbars that I have installed - as I have read that certain toolbars have caused issues in the past - The sound thing doesn't seem to be a/the issue on this machine as I did have the code running without the call to the server - the code seems fine on Firefox also.  When the crash occurs there is an icon on the bottom right of the browser that I have previously seen in relation to managed addins - however I'm not using any add ins - the bgsound I tested by using the sample page at http://www.phon.ucl.ac.uk/home/mark/audio/play.htm.

To clarify  I have the following toolbars on this machine :

Standard Buttons  IE standard
Address Bar IE standard.
Links   (not activated) IE standard
Interwoven Visual Annotate    Content Mangement Solution - 3rd party
Developer Toolbar   Disabled by security policy - Microsoft
Hummingbird DM    3rd party - Document management System

The videos don't show the actual error message. 

I think what needs to be done is isolate what it is about this machine's IE that is causing the issue.  If you can browse the page successfully on another machine with IE without any of those addins then you know it is not an IE only issue.  Then I'd add each toolbar one at a time, verifying that the page still works.  Once you narrowed down the troublesome addin we can try and determine what action to take (perhaps see if the addin has an update).
