I see on a lot of DNN sites a registration that has more fields than the out-of-the-box registration page (with city/state, etc). Is this a setting or a custom module that I'm seeing?

Most likely it is a custom registration module. The one we use on our websites is UCanUse (www.ucanuse.com) also I know there is one from DataSprings which integrates with a few of their other modules. The one from UCanUse is very good, we added a few fields to the form such as "How did you find us" and I use the module to place members in certain security groups based on their answers and can direct them to a specific page after they are done.

In my recent investigations of DNN, I've had to register on a lot of sites, and it seems like the majority of them had a very similar, if not identical, enhanced registration (i.e. more than just name). I wasn't sure if there was just a site-setting to get these extra fields, or if it was a custom module...

You can add fields to the user profile, no custom module needed.  However, I've found that aside from a simple textbox or checkbox, the functionality is quite limited or looks like it should work but doesn't.

One word of caution - if you add a field and make it "required", all of your current users will have to update their profile when they attempt to log in.  It's a good thing that it works this way, but it came as a shock to one of my clients.
